What They’re Doing : Community based chronic disease management and peer support for patients with severe mental illness.
Clinical Innovation : Provides chronic disease-specific preventive care and management in the community setting for patients with serious mental illness with a focus on cardiovascular disease, cancer, stroke, pulmonary conditions, accidents, and diabetes. The patient receives a care plan and is connected with a staff member who offers the patient care management, health education, support for lifestyle changes through motivational interviewing and mindfulness training and guidance on adherence to the care plan. The program also offers health coaching and counseling, peer support and exercise classes.
Evaluation Type : Currently Non-Experimental/ Qualitative Support ; 2007-2010 Experimental
Evaluation Plan : Created research protocol to formally study the health status and behaviors of clients as well as effectiveness of program, data analysis.
Patient Health and Cost Outcomes : Individual results have been variable; most individuals have made significant health gains through weight loss, smoking cessation, improved lipid and hypertension control and improved diabetes management.
