Experience Workgroup

Maryland Health Disparities Collaborative

Objective: Improve health and health care outcomes for racial and ethnic minorities and underserved populations and communities

Provide a set of evidence-based options for communities and local health institutions and health plans to consider in establishing effective community health proposals for Health Enterprise Zones. These options may be made available to applicants for guidance in putting together proposals.

Make recommendations for successful models of care that address disparities that are appropriate for inclusion at www.dhmh.maryland.gov/innovations.

Make recommendations for the format of reporting of efforts on health disparities by hospitals.
