SALARY: $22,448 - $34,716 (Grade 6)

DESCRIPTION: This position is located within the Health Department’s Bureau of Prevention, Wellness and Recovery. The main purpose of this position is to provide and assist the Coordinator with recovery support responsibilities and the reintegration of patients into long term recovery at multiple sites throughout the community. Job duties include: learning to proved health care and disease prevention information to individuals under the direction of the Care Coordinator Manager; transporting patients to medical treatment appointments, court appearances, interviews and continuing care facilities for further treatment as needed; and learning to contact the local providers that patients may need in the community and making connections with them to provide services. Working hours may vary depending on the needs of the program. Applicants who have successfully completed an addictions or co-occurring disorder recovery program are encouraged to apply.
MINIMUM QUALIFICATIONS:
EDUCATION: Graduation from an accredited high school or possession of a high school equivalency certificate.
EXPERIENCE: None
NOTE:
Experience providing assistance to individuals in a health care or social services setting may be substituted for the required education on a year-for-year basis.
LICENSES, REGISTRATIONS AND CERTIFICATES:
Employees in this classification may be assigned duties which require the operation of a motor vehicle. Employees assigned such duties will be required to possess a motor vehicle operator's license valid in the State of Maryland.
