Divorce Verification

The Following Persons May Apply:

  • Either spouse named on the record
  • Representative of either spouse named on the record. (Note: The representative must show a letter stating that s/he has permission to obtain a copy of the Verification of Divorce. The letter must be signed by the either spouse named on the record and certified by a notary public.)
  • Attorney representing either spouse named on the record (Note: The attorney must present proof that s/he represents either spouse named on the record.)

The Division of Vital Records verifies divorces and annulments that occurred on or after January 1, 1992. The Verification of Divorce form includes the names of the former spouses, place, date, and type of divorce action and is accepted by certain agencies seeking proof of divorce or annulment. Please note that a copy of the actual Decree of Divorce and Annulment of Marriage can only be obtained from the Circuit Court that granted the decree.

A non-refundable fee of $12.00 is required for each verification requested. You may apply in person at the Division of Vital Records located at 6550 Reisterstown Road, Baltimore 21215, or by mail. You must present a valid, unexpired, government-issued photo ID displaying a date issued and an expiration date. Applicants unable to supply valid photo ID must present two (2) different pieces of alternative documentation. Acceptable documents are a pay stub, current car registration, bank statement, letter from a government agency, lease/rental agreement, utility bill with current address, or a copy of your income tax return or W-2 form. At least one of these documents must contain your current mailing address. Applicants unable to provide valid photo ID will not be able to obtain verification of a divorce the same day. The verification form will be mailed to the address displayed on the documents provided.

When applying by mail, please enclose the completed application (see links above), copies of required identification, fee, and a self-addressed, stamped envelope. If you cannot download the application, you may provide the information listed below on a separate sheet. The turnaround time for mail received is 3-4 weeks.

  • Full maiden name of wife named on the record (first/middle/last)
  • Full name of husband named on the record (first/middle/last)
  • Date of divorce/annulment (month/day/year)

You Will Receive:

You will receive a Verification of Divorce form that indicates the names of the persons divorced, the type of court action, the county where the divorce took place, and the date of the decree. Note that a copy of the Decree of Divorce and Annulment of Marriage can only be obtained from the circuit court granting the decree.
